I understand the dates I select are final and will not be switched for any reason. There will be no refunds on any reserved weeks. *

I undertsand all camp weeks are subject to cancelation due to low enrollment.*

I hereby consent to my childs participation in the Titan Elite Summer Camp Program. I acknowledge that activities involving height or motion including dance, trampolines, gymnastics, and tumbling, carry risks that may result in serious injury, paralysis, or death. I release Titan Elite, its owners, employees, and coaches from all liability for injuries or damages occuring while my child is under their instruction or supervision. I agree to be held responsible for any medical expenses incurred.

I give my permission to trained medical professionals to administer emergency medical treatment to my child should sickness or accident occur in my absence.*
